The Clinical Pharmacist functions at the highest level of clinical practice, working independently under a scope in the ambulatory care practice setting.  The Clinical Pharmacist is fully responsible for planning, developing and maintaining highly innovative and progressive clinical pharmacy programs in their assigned area by implementing rational cost effective drug therapy to improve patient outcomes.
Primary Responsibilities:
Perform comprehensive medication review covering all prescription and nonprescription drugs, herbs, and supplements taken by the patient to identify medication-related problems:
  • Perform medication reconciliation: Check for discrepancies between electronic records and patient’s supply
  • Confirm that the patient is compliant and assess barriers to adherence
  • Evaluate the appropriateness, effectiveness and safety of the patient’s medications
    • Evaluate if evidence based medicine and standards of care have been followed
    • Identify complex regimens that can be simplified;
  • Identify adverse events;
  • Identify pharmacotherapy duplication
  • Identify drug-drug, drug-herb, and drug-disease interactions
  • Assess the status of the patient’s health problems and determine whether the prescribed medications are optimally meeting the patient’s needs and goals of care
  • Advise the patient on how to best take the medications
  • Recognize untreated health problems that could be improved or resolved with appropriate medication therapy
  • Follow the patient’s progress to determine the effects of the patient’s medications on their health
    Develop Care Plan
  • Develop care plan with individualized therapy goals and personalized interventions
  • Design and provide personalized education programs. Educate patients about managing their conditions. Counsel patients about proper medication use. Educate patient about each medication purpose, dosage, timing and side effects. Provide educational materials to patient
  • Develop a prioritized list of problems (medications being taken but not indicated; medications indicated but not prescribed; patient adherence issues; supra therapeutic drug dosages; generic, formulary, or therapeutic substitution issues
  • Monitor patients according to key indicators. Review electronic record activity routinely for Adverse Event and side effect indications. Monitor to ensure adherence to established procedures and to identify outcomes. Follow-up with patient
  • Check lab values
  • Establish therapeutic goals.  Discuss goals of therapy with patient and provider
  • Initiate, modify, discontinue drug therapy/dosage  and order lab tests related to a patient’s medical condition according to established protocols and collaborative practice agreements
  • Coordinate care follow-up with physician and disease management program.  Refer patients to chronic disease management, Transitions in Care and other targeted ACO programs when appropriate
  • Communicate with payers to resolve issues that impede access to medications
  • Schedule follow-up appointments
  • Document outcomes. Document when goals of therapy met
  • Complete detailed assessment forms, care plan, and evaluation forms. Document in electronic health record
Required Qualifications:
  • Graduation from an accredited College of Pharmacy with a B.S. with 10 years of clinical experience or Pharm.D. degree
  • Completion of an ASHP accredited PGY1 pharmacy residency
  • Connecticut Pharmacist License
Preferred Qualifications:
  • Certification by the Board of Pharmaceutical Specialties
  • Completion of a disease state managementcertification program approvedby the Accreditation Council for Pharmacy Education
  • Experience in Managed Medicare pharmacy (e.g. ,Pharmacy Benefits manager, clinical related programs, appeals, prior authorization)
  • Knowledge of Medicare Part D
  • Knowledge of MS Outlook, Word and Excel
  • Experience with electronic health record
